Medicare & Chronic Disease Management (CDM)
Medicare’s CDM program assists patients with chronic and complex medical conditions. With a GP referral, you are eligible for a rebate of $60.35 per session, up to 5 sessions per year. Rise Health Group accepts CDM plans for Podiatry, Physiotherapy, and Exercise Physiology.
WorkCover
Rise Health Group offers comprehensive care for individuals recovering from workplace injuries. With expertise in exercise rehabilitation, we provide a clear recovery roadmap and encourage use of our fully equipped gym and clinical Pilates studio. Please inform our staff when booking for WorkCover to ensure direct billing.
Transport Accident Commission (TAC)
We specialise in treating patients with motor vehicle-related injuries. Our practitioners provide tailored rehabilitation programs, taking into account age and lifestyle factors. For TAC claims, please provide your TAC claim number and case manager details when booking.
Veterans Affairs
Rise Health Group offers gap-free services for patients with a DVA referral. We work closely with the Department of Veterans’ Affairs to support veterans, war widows, and their families. Please inform our staff of your DVA cardholder status and bring your card to your first appointment.
